Consular Visit to Memphis - August 21

Consul Michal Sikorski from the Consulate of the Republic of Poland in Washington D.C.,
will visit Memphis in August to provide consular services to the Polish community of
the Mid-South region.
Here is the agenda of the Consul’s visit:
Saturday, August 20, 2011
11 am - 5 pm - Germantown International Festival.
-- Consul Sikorski will join our team to represent the Memphis Polonia
at the Festival. Please visit the Polish American

After 7 pm Polish Mass - Family Life Center at St. Ann Catholic Church, 6529 Stage Road,
Bartlett, TN 38134
Meeting with Consul (it is important that all those who made their appointments with the
Consul attend this meeting)
– Information about the schedule of individual visits with the Consul, Q&A regarding
passport and other consular matters.

Please remember that the requests for a meeting with the Consul must be made in advance by
contacting the Consulate via email to
washington.consular@msz.gov.pl or by calling
(202) 234-3800 (ext. 2201). Full consular services will be provided. The Consular Division
asks all applicants to ensure they bring all documents in support of your application -
originals and copies. Also, please review all passport-related information available on the
embassy's website - special attention should be paid to requirements for passport pictures.
If you have any questions or require assistance, please use the contact information above
to get clarification from the consulate. You need your PESEL number to apply for the Polish
passport. If you don't have a PESEL number, you need to advise the consulate when making an
appointment.
